Permit Application Form
Instructions: Applying for a permit is a multi-step process. All the following steps must be completed before a permit may be issued:
- Check to make sure you can meet the requirements for submittal as specified here.
- Use the permit calculator at the top of this application to select the type of permit you need and calculate the charges. After all information is filled out, click "Submit" at the bottom.
- Pay any fees by using the link on the page presented after the application. You can also check your applications and pay at any time using the Permit Portal.
- Submit your plans and any other required documentation electronically using the above link. You may also deliver them in person to 1110 WEST WASHINGTON, SUITE 100, PHOENIX, ARIZONA 85007.
NOTE: A FIRE CODE REVIEW FOR GENERAL CONSTRUCTION IS REQUIRED BEFORE ANY FIRE PROTECTION
SUB-SYSTEM SUBMITTAL. FAILURE TO PROVIDE ACCURATE AND COMPLETE INFORMATION ON THIS APPLICATION
WILL RESULT IN THE PERMIT APPLICATION BEING REJECTED.

List all Other Jurisdictions involved in this Project (Fire Departments, Bldg. Departments, etc.) If this project is on Sovereign Nation Land, specify here.
In accordance with the 2003 IFC, all permits issued shall automatically become invalid
if the work authorized by issued permit is not inspected within 180 days.
